Please create a thread dump while Minecraft is frozen.
The only simple way of doing so, that I can think of, is to use the old FTB launcher which has a button for creating a thread dump (installing just the ReplayMod and Forge won't be easy though). Other launchers might also provide that feature but I'm unaware of any that do.
Otherwise, you'll have to do some research on how to do that on Windows.
